You must report the injury to your campus nurse as soon as possible.
If a nurse if NOT available, report the injury to your immediate supervisor.
If your supervisor is not available, please go to https://www.publicschoolworks.com/ and file an injury report.
If you need to seek medical attention:
You MUST see a doctor that accepts workers compensation. If you do not, you may be responsible for all medical charges. Personal medical insurers may deny your claim. Most primary care physicians will not treat workers compensation injuries.
You MUST notify Jessica Morris of when you went to the doctor, where you went, and send in any documentation that you received from the medical facility.
You MUST inform the doctor's office of the districts workers compensation carrier and their billing address.
If you miss work due to a work injury, notify Jessica Morris immediately. Please note that the district MUST receive a DWC Form 73 (Work Status Report) following each doctor’s visit for the specific injury. (The doctor’s offices are familiar with this form.)
Paid leave - Temporary Income Benefits (TIBs) - are not effective until the 8th day of absence for a specific injury. Therefore, if you have any absences as a result of this injury, we will need you to complete a standard form designating the number of available leave days you wish to use, in lieu of being docked. If a selection is not made, CISD is required to automatically list the absences as non-paid.
